How to Find a Primary Care Provider (PCP)

How to Find a Managed Care Plan

You may opt out of your Health First Colorado managed care plan for any reason during the first 90 days of your enrollment. You can also opt out each year during your open enrollment, which is the two months before your birth month. You can opt out of your managed care plan at any time if you have good cause. Some examples of good cause might be:

  • You move out of the managed care plan’s area
  • The managed care plan does not cover the service you need
  • You received poor quality care in the managed care plan you’re in now
  • You can’t find a provider to treat your problem
  • Health First Colorado Enrollment made a mistake and put you in the wrong managed care plan

If you leave your managed care plan, you will need to choose a PCP. If you know the PCP you want, go to Choose. If you’re not sure who you want as your provider, go to Find a Provider.

Child Health Plan Plus (CHP+)

How to Find a Managed Care Plan

You may opt out of your CHP+ managed care plan for any reason during the first 90 days of your enrollment. You can also opt out each year during your redetermination and enrollment period. You can opt out of your managed care plan at any time if you have good cause. Some examples of good cause might be:

  • You move out of the managed care plan’s area
  • The managed care plan does not cover the service you need
  • You received poor quality care in the managed care plan you’re in now
  • You can’t find a provider to treat your problem
  • CHP+ made a mistake and put you in the wrong managed care plan
